As a client of Hallam Pilates we want to reassure you that we treat your details very carefully and data with the up most respect.
We will protect any personal information you provide us with. This information can be obtained through making an appointment, the clinical examination, Pilates class or from any purchase of products.
Any changes to the policy will be made by updating this page. You should check this page from time to time to ensure that you are happy with any changes. This policy is effective from 01.05.18.
What personal data we collect
– Name, date of birth and address
– Contact information such as a telephone number and/or an email address
– History of your health, your family’s health and any related conditions you have, your medication, occupation and interests.
– Gp or consultant name and address
– How your treatment is paid for e.g. self-payer – private insurance policy – referral from a third party.
This information will primarily be collected from you with your permission.
Where it is lawful to do so, we may collect information from other sources such as the NHS or other healthcare providers authorised to provide it. This includes parents or carers who have permission to give information on your behalf.
How we use your data
When you attend your first you will be asked to complete a paper registration form. This allows the Physiotherapist to gather the relevant information from you prior to your appointment ie name , date of birth , address , contact details, Gp details / consultant, private health provider or self-pay. The purpose of this information is to allow your clinician to ensure they manage your case with relevant due care and consideration for your age and requirements.
We will only use the data for the purpose it was collected. We do not share your data with any other third party. If relevant to share your Physiotherapy details with other Healthcare professionals caring for you as part of a multi- disciplinary approach to your care, or employer this can occur only with your prior consent
How we store your data
This data is stored on a secure server PPS as part of our clinical data base. Under GDRP guidelines we are the data controllers and so have a responsibility to ensure that your data is processed securely. All electronic data is therefore password protected and only accessible by authorised individuals. Your clinical notes are in paper form and are stored securely for a period of up to 8 years as required by law.
How to update your information

Protecting personal information
We are committed to ensuring that your information is secure. In order to prevent unauthorised access or disclosure, we have put in suitable physical, electronics and managerial procedures to secure the information we collect from you. All paper copies of your records are destroyed after 8 years and your data electronically accessed only by our data processors.
We cannot 100% guarantee the complete security of electronic database however we do have anti-virus soft wear in place. We can also not 100% guarantee that the information you supply to us over the Internet will not be intercepted, this is at your own risk.
